A BIT ABOUT ADULT CARE MANAGEMENT

Adult Care Management, Inc. (ACMI), is a 501(c)3 not-for-profit agency that provides case management to individuals participating in Long Term Support Service (LTSS) Medicaid programs in four Colorado counties.

 

ACMI has served participants on the Colorado Medicaid program for over two decades & maintains a contract with the state of CO to provide these services.

 

ACMI's primary function is to assess individuals for LTSS programs and to help develop & implement service plans to help individuals stay in a setting of their choice. 

Although the "A" in ACMI stands for Adult, ACMI also works with children from 0-18 yrs of age.

ABOUT ACMI, IMAGINE! AND COLORADO'S CMRD INITIATIVE

ACMI, as an existing Single-Entry Point agency, and Imagine!, as an existing CCB, are partnering together to fulfill Colorado's Case Management Redesign initative. 

We will make our formal transition into a single Case Management Agency (CMA) serving Boulder, Broomfield and Gilpin counties on July 1, 2024.

CO LTSS Program Options

There are 11 Colorado Home & Community Based Waiver's and ACMI provides case management for 5 of them. ACMI also manages one state only program called Home Care Allowance (HCA), and ACMI conducts assessments for individuals seeking a long term stay at a Medicaid certified nursing facility.

 

Common examples of services provided by these various programs include, but are not limited to: personal care and homemaker services (agency and consumer-directed options), medication dispensers and lifeline buttons, home modifications, transportation, transition support services for people coming out of nursing facilities and other restrictive environments, assisted living services, independent living skills training, case management, 24-hr care offered by nursing facilities, and others.  

 

In ACMI's coverage area we specifically manage the: 1) Elderly Blind and Disabled Waiver, the 2) Community Mental Health Supports Waiver, the 3) Traumatic Brain Injury Waiver, 4) the Children with Life-Limiting Illness Waiver, 5) the Children's HCBS Waiver, and the 6) Home Care Allowance Program.
